Before You Submit a Report
Please provide as much information as possible. Anonymous reports or reports with limited details may take longer to review. You may include supporting documents or other attachments with your report.
Reports are handled in accordance with FERPA. Because reports may become part of a student’s educational record, the student may have the right to access them.

CARE Team Report
Are you concerned about a student who may be struggling or displaying concerning behavior? Submit a CARE Team Report. The CARE Team will review the concern and connect the student with appropriate support and resources.
Student Code of Conduct Report
Use this form to report a possible violation of the Student Code of Conduct involving a student or recognized student organization, either on or off campus.
Examples may include alcohol or drug misuse, disruptive behavior, physical assault, theft, organizational misconduct, or other violations of college policy.

General Student Complaint
Use this form if you believe an action by a college employee, department, or college-affiliated group violated a WIT rule, policy, or procedure.
Before submitting this form, you must complete Steps 1 and 2 of the **General Complaint Procedure** outlined in the WIT College Catalog.
This form should not be used for grade appeals or concerns involving harassment or discrimination. Please follow the applicable process listed in the College Catalog, Student Handbook, or college website.

Harassment, Discrimination, or Title IX Report
Students may use this form to report harassment, discrimination, sexual misconduct, or other Title IX-related concerns involving a WIT student.
Concerns involving employees should be reported directly to the WIT Human Resources Office.

Campus Safety Incident Report
Use this form to report a non-emergency safety or security incident to the WIT Campus Safety Office.
