There appears to be a discrepancy with your parent's reported social security number(s), Date of Birth and/or Name on the Free Application for Federal Student Aid (FAFSA). Please provide a copy of your parent(s) social security card and correct date of birth to the Financial Aid Office by fax at 712-274-6470 or drop if off in Financial Services located on campus.
